• ID OR PASSPORT (CLEAR COPY)(MARRIGE CERTIFICATE if applicable)

• 2 x CURRENT WRITTEN reference letters (signed as confirmed/date)

• COPY OF CERTIFICATES (School/College/University)

• ALL OTHER CERTIFICATES OF COURSES DONE,

ATLS, ACLS, PALS, BACHELOR OF SURGERY ETC.

• DISPENSING COURSE DONE AND RECEIPT

• HPCSA/SANC/SAPC REGISTRATION CERTIFICATE

• MPS (MALPRACTICE INSURANCE COVER)
